Application

Doggie Oasis requires a four hour evaluation for all new dogs. This enables us to know if Doggie Oasis is the right fit for your dog. This application is our pre-screening form. We will contact you to make an evaluation appointment if your dog qualifies. There will be more paperwork to fill out at the evaluation. We like to know as much about your dog as possible! If you have multiple dogs with very different personalities, please fill out the form more than once, or let us know in the notes section to ask you about your other dog(s)! You may also call us at 734-DOGS with any questions, or to have us fill this out over the phone. There is an $18 charge per dog for the evaluation. This is the same as our 1/2 day rate for four hours or less.
